NDMA STUDENT APPLICATION-fillable All students are required to apply for enrollment.
NDMA Transportation Form Turn it in to the office, mail it, or email it to [email protected].
- Please note that students may be dropped off no earlier than 8:00 a.m. There is no supervision before that.
- Please remember when dropping off or picking up students that you always have them exit/enter the vehicle on the sidewalk side. Cars, buses driving through, morning and afternoon, ice patches – these all create a hazard for your child if they are not on the sidewalk side of your vehicle.
- Please remember to call the office, 320-234-6362 x1, to report your student’s absence or change in the pick-up schedule.
There is a non-refundable registration fee of $25.00 with the application.
3-year-old by September 1 option: $300.00/month (scholarship applications available)
$300.00/month for morning & noon transportation (within Hutchinson city limits)
Tuesday, Wednesday, Thursday 8:15 a.m. – 12:00 p.m. (half-day) Includes breakfast, and lunch.
4-year-old by September 1 options:
$0.00/month. No charge.
Monday, Tuesday, Wednesday, Thursday 8:15 a.m. – 2:50 p.m. (full-day) 8:15 a.m. – 12:00 p.m. (half-day by request only)
Includes: breakfast, lunch, morning transportation, afternoon transportation

Dear Families,
In efforts to match State compliance, any child needing or requesting a special diet that deviates from the school menu will be required to have a Special Diet Statement Form on file in the school kitchen office. There are two Special Diet Statement Forms. The first form is for participants with a disability that affects the diet. This would be for children whose non-altered diet would be life-threatening due to an anaphylactic reaction. The second form is for participants without a disability. This would be for children with allergies or intolerances requesting a special diet. Please read the form carefully, as both forms have areas for parents/ guardians and certified medical personnel to fill out. These forms must be filled out completely. It will not be necessary for future years to have a new form filled out unless there have been changes requested as the Special Diet Forms will remain on file for the remainder of your child’s NDMA school years.
